ECMC’s Family Health Center First ECMC Primary Care Practice to Receive Certificate of Recognition from NCQA and NYS
ECMC’s Family Health Center is the first of ECMC’s primary care practices to receive Certificate of Recognition from National Committee for Quality Assurance (NCQA) and New York State for “systematic use of patient-centered, coordinated care management processes and transformative quality.” The Center is also the first to successfully execute NCQA’s new annual reporting model, differing from the prior year’s triannual recognition structure.
“On April 1, 2018, The New York State Department of Health (NYSDOH) released an innovative model for primary care transformation known as the New York State Patient-Centered Medical Home (NYS PCMH).
