Medicine can be most successful when there is care for all stages of life and every kind of issue. A number of our Primary Care Centers are NCQA Patient Centered Medical homes, which means they offer care for the whole person.

ECMC Primary Care Centers are committed to delivering whole person care for all stages of life: acute care, preventative care, chronic care, and end-of-life planning. Your Primary Care Physician will ensure all your needs are addressed, including behavioral health needs, which are available via local partners, or through expedited services at ECMC Downtown Clinical Services at 1285 Main Street in Buffalo.
Our centers also provide self-management support to our patients—giving them the tools, training, and education to successfully manage their own health.

- Offer services in a timely manner
- Get to know you, your family situation, and your preferences for care

- Ensure that you have time to ask questions and get the answers you need
- Ensure that your care is evidence based
The Role of the Patient
- Provide complete health history including the health of your family
- Tell your provider about all the prescription and non-prescription medication you are taking
- Notify us of other doctors you have seen recently, including any ER visits or hospital admissions
- Take an active role in your health
- Follow the care plan given to you by your doctor
- If you are struggling with your care plan, tell us about it
- Complete your records transfer form prior to your first appointment at our office
